Melanomas are malignant tumors derived from neuroectodermal melanocyte cells. In Poland, a steady increase in incidence is observed, with the number of newly registered cases reaching 4,500 per year. Advances in early diagnosis and the introduction of modern systemic treatment methods, based on immunotherapy and targeted therapy, have contributed to an increase in 5-year relative survival by approximately 20% by 2017.


The fifth part of the publication "Melanoma. Modern Treatment" focuses on the modern principles of surgical treatment, with particular emphasis on sentinel lymph node biopsy, perioperative and systemic treatment in the generalized stage. It is supplemented by case descriptions with expert commentary and the current topic of systemic therapy in patients with uveal melanoma, where tebentafusp is available.


The possibilities of perioperative treatment in stage IIB and IIC after a negative sentinel lymph node biopsy have been expanded by the introduction of reimbursed treatment with pembrolizumab or nivolumab. In adjuvant treatment, the risk of disease recurrence and the expected benefit of systemic treatment should be carefully assessed based on the results of NNT and NNH analysis, especially for patients with a lower risk of recurrence, i.e. in stage IIIA and IIB. It is also necessary to introduce preoperative treatment with combined immunotherapy in patients with clinical metastases in stage III, which is clinically and pharmacoeconomically justified.
