The monograph discusses in detail the development of the PHC concept, emphasizing the importance of multidisciplinary collaboration, new competencies of medical personnel, and effective models of care coordination. It points to the key role of the Primary Health Care Act, which created the foundations for the implementation of a coordinated care model at the PHC level. A key element of the analysis is the role of the care coordinator, a new function in the Polish health system, which aims to improve access to services and the efficiency of treatment.

The empirical part of the monograph refers to the POZ PLUS program, in which coordinated care was tested, and its impact on systemic changes. An undeniable value of this work is the choice of the analyzed problem.
