The monograph discusses in detail the development of the PHC concept, emphasizing the importance of multidisciplinary collaboration, new competencies of medical personnel, and effective models of care coordination. It points to the key role of the Primary Health Care Act, which created the foundations for the implementation of a coordinated care model at the PHC level.
